Good Faith Estimate

Your right to a Good Faith Estimate

Under federal law (the No Surprises Act), you have the right to receive a Good Faith Estimate explaining how much your medical care will cost.

Health care providers must give patients who don't have insurance or who aren't using insurance an estimate of the bill for medical items and services upon request, or in advance of scheduling an appointment.

What you have the right to receive

  • A Good Faith Estimate of expected charges for medical items and services in writing at least one business day before your service
  • The right to ask your health care provider for a Good Faith Estimate before scheduling a service
  • The right to dispute the bill if it is at least $400 more than the Good Faith Estimate

How to request a Good Faith Estimate

Call us at (817) 518-3165 or visit us at 104 Grapevine Hwy, Suite 400, Hurst, TX 76054 to request a written Good Faith Estimate. We'll provide one within the timeframes required by law.

If you have insurance

If you have insurance and we're billing your insurance directly (PIP, MedPay, health insurance, or letter of protection), the Good Faith Estimate process generally does not apply, as your charges are managed through your insurer's coverage and our billing agreement.
